New Patient
How to Become our Patient?
We offer you two options: Online or In Person:
Online
- Fill out the online Healthcare Provision Agreement
- You can send the signed agreement to us by email or deliver it in person to the clinic.
- We will arrange the transfer of your medical records.
In Person
- Come in person with your ID card, no appointment needed:
Monday – Tuesday 7:00 – 15:00
Wednesday 12:00 – 17:00
Thursday – Friday 7:00 – 14:00
- At the reception, we will help you fill out the Healthcare Provision Agreement.
- We will arrange the transfer of your medical records.
Changing Doctors? Don’t Worry, We Will Handle Everything for You.
After signing the agreement, we will take care of everything else – communication with your previous doctor, your insurance company, and the transfer of your medical documentation.
Requirements for Signing the Agreement
Anyone can enter into the agreement who:
- has valid public health insurance in Slovakia
- has reached the age of 18
- has been registered with their previous general practitioner for at least 6 months
- or has been granted an exception by agreement with our doctor.
