New Patient
How to become our patient?
We offer you two options: online or in person:
By email
- Fill out the Healthcare Provision Agreement
- You can deliver the signed agreement to us by email or in person at the clinic
- We will arrange the transfer of your medical records
In person
- Walk in with your ID card—no appointment required:
Monday – Tuesday 7:00 – 15:00
Wednesday 12:00 – 17:00
Thursday – Friday 7:00 – 14:00
- We will fill out the Healthcare Provision Agreement with you at the reception
- We will arrange the transfer of your medical records
Changing doctors? No worries, we will handle everything for you.
After signing the agreement, we will take care of everything else – communication with your previous doctor, the insurance company, and the transfer of your medical records.
Conditions for signing the agreement
The agreement can be concluded by anyone who:
- has valid public health insurance in the Slovak Republic
- has reached the age of 18
- has been registered with their previous general practitioner for at least 6 months
- or has been granted an exception upon agreement with our doctor
